ABSTRACT

Objetivo: descrever o processo de condução de um caso com manifestação oftálmica pela mpox, destacando o trabalho da enfermagem na prevenção de complicações da doença. Método: relato de experiência da condução de um caso de mpox ocorrido em setembro de 2022 com manifestação oftálmica. Resultados: a pronta identificação da complicação oftálmica, com a presença de conjuntivite e edema palpebral, apresentada por um paciente com suspeita de mpox pela equipe de enfermagem da vigilância epidemiológica durante as ações de monitoramento, e o envolvimento da equipe com outras equipes de diversos níveis da assistência à saúde permitiram a instituição precoce do tratamento com antiviral recomendado pelo Ministério da Saúde, o que contribuiu para um melhor desfecho. Conclusão: é importante que se dissemine o conhecimento sobre as manifestações oftálmicas associadas à mpox para que a equipe de enfermagem, que atua nos diversos níveis de atenção à saúde, esteja atenta para implementar, de forma precoce, medidas de prevenção, diagnóstico e tratamento adequados.


Objective: to describe the process of conducting a case with ophthalmic manifestation by mpox, highlighting the work of nursing in preventing complications of the disease. Method: experience report of managing a case of mpox that occurred in September 2022 with ophthalmic manifestation. Results: the prompt identification of the ophthalmic complication, with the presence of conjunctivitis and eyelid edema, presented by a patient with suspected mpox by the epidemiological surveillance nursing team during monitoring actions and the team's involvement with other teams from different levels of the health care allowed the early initiation of antiviral treatment recommended by the Ministry of Health, which contributed to a better outcome. Conclusion: it is important to disseminate knowledge about ophthalmic manifestations associated with mpox for the nursing team that works at different levels of health care to be attentive to implement preventive measures, diagnosis and adequate and early treatment.


Objetivo: describir el proceso de manejo de un caso con manifestación of- talmológica por mpox, destacando la labor de enfermería en la prevención de complica- ciones de la enfermedad. Material y método: relato de experiencia de manejo de un caso de mpox ocurrido en septiembre de 2022 con manifestación oftálmica. Resultados: la rápida identificación de la complicación oftálmica, con la presencia de conjuntivitis y edema de párpados, presentada por un paciente con sospecha de mpox por el equipo de enfermería de vigilancia epidemiológica durante las acciones de monitoreo y la partici- pación del equipo con otros equipos de diferentes niveles de la asistencia sanitaria per- mitió el inicio precoz del tratamiento antiviral recomendado por el Ministerio de Salud, lo que contribuyó a un mejor resultado. Conclusión: es importante difundir el conoci- miento sobre las manifestaciones oftálmicas asociadas a la viruela del mono para que el equipo de enfermería que actúa en los diferentes niveles de atención a la salud esté atento a implementar medidas preventivas, diagnósticas y de tratamiento adecuado y precoz.

RESUMO Objetivo: Avaliar sintomas astenópicos e fatores sociodemográficos, hábitos comportamentais e clínicos nos docentes universitários durante a pandemia da COVID-19. Métodos: Trata-se de estudo transversal que avaliou a ocorrência de astenopia em 104 docentes. Questionários de sintomatologia visual validados foram adaptados para a coleta de dados. Houve comparação dos docentes quanto à ocorrência ou não de sintomas astenópicos, bem como foi aplicada regressão logística binária, para aferir a associação com variáveis independentes (p<0,05). Resultados: Houve maior aparecimento de sintomas astenópicos durante o período pandêmico, em que o tempo de exposição a telas parece ter sido o fator mais determinante. Além disso, os indivíduos com tempo de tela superior a 5 horas diárias, que faziam uso de telas para o lazer e usavam colírio/lubrificantes apresentaram significativamente maior chance de estar no grupo com sintomas astenópicos. Conclusão: Foi identificada associação significativa entre a ocorrência de sintomas astenópicos e o uso de telas durante o período pandêmico, principalmente nos grupos com maior duração do tempo de tela. O estudo chama atenção para a saúde ocular de docentes universitários em ensino remoto, além de suscitar novos estudos para investigação desse quadro em distintos ambientes escolares.


ABSTRACT Objective: To assess asthenopic symptoms and sociodemographic factors, behavioral and clinical aspects in college lecturers during the COVID-19 pandemic. Methods: This is a cross-sectional study evaluating asthenopia in 104 lecturers. Some validated visual symptom questionnaires were adapted for data collection. Lecturers were compared regarding the occurrence or not of asthenopic symptoms, and binary logistic regression was applied to measure the association with independent variables (p<0.05). Results: Asthenopic symptoms occurred more often during the pandemic, when exposure to screens was a determinant factor. The individuals with screen time longer than five hours a day, who used screens for leisure, and who used eye drops/lubricants were significantly more likely to be in the group with asthenopic symptoms. Conclusion: A significant association was identified in occurrence of asthenopic symptoms and screen use during the pandemic period, especially in groups with longer screen time. The study draws attention to the eye health of college lecturers in distance learning, and the need for further research on this situation in different school environments.

RESUMEN El objetivo de este estudio fue determinar manifestaciones oculares de la toxocariasis en escolares. Se realizó un estudio en dos escuelas del estado Anzoátegui en Venezuela en el 2019. Se empleó la prueba de ELISA para determinar los anticuerpos IgG contra Toxocara spp. Las familias completaron un cuestionario y los niños fueron evaluados clínicamente por pediatras y oftalmólogos. Participaron 118 niños, el 18,6% presentó anticuerpos anti-Toxocara spp. Las manifestaciones clínicas con asociación estadísticamente significativa fueron las reacciones alérgicas, epífora y disminución de la agudeza visual. En la evaluación oftalmológica se encontró queratitis, uveítis, iritis, granuloma retiniano, endoftalmitis, amaurosis, leucocoria, desprendimiento de retina y endotropía. Los hallazgos muestran una alta frecuencia de enfermedad ocular en niños con toxocariasis de un estado de Venezuela.


ABSTRACT The objective of this study was to determine ocular manifestations of toxocariasis in schoolchildren. A study was conducted in two schools in the Anzoátegui state in Venezuela in 2019. The ELISA test was used to determine IgG antibodies against Toxocara spp. The families completed a questionnaire, and the children were clinically evaluated by pediatricians and ophthalmologists. 118 children participated, 18.6% presented anti-Toxocara spp. The clinical manifestations with a statistically significant association were allergic reactions, epiphora, and decreased visual acuity. The ophthalmological evaluation found keratitis, uveitis, iritis, retinal granuloma, endophthalmitis, amaurosis, leukocoria, retinal detachment and endotropia. The findings show a high frequency of eye disease in children with toxocariasis from a state of Venezuela.

ABSTRACT

El epitelio corneal es una importante barrera de defensa que impide el ingreso de una gran variedad de microorganismos. Cualquier alteración de la superficie ocular facilita la invasión bacteriana de la córnea. El germen más frecuentemente identificado es Staphylococcus aureus. Se presenta una paciente con enfermedad debida al virus de la inmunodeficiencia humana (VIH) con diagnóstico de sida, absceso corneal bilateral y lesiones cutáneas. S.aureus meticilino resistente se aisló en hemocultivos y en material obtenido por raspado de la córnea. El absceso corneal es una entidad poco frecuente en pacientes con infección por VIH y síndrome de inmunodeficiencia adquirida.


The corneal epithelium is an important defense barrier that prevents the entry of great variety of microorganisms. Any alteration of the ocular surface facilitates bacterial invasion of the cornea. The most frequently reported germ is Staphylococcus aureus. Here, we present a patient with a diagnosis of HIV/ AIDS disease, who developed bilateral corneal abscess and skin lesions. Methicillin-resistant Staphylococcus aureus was isolated from blood cultures and corneal scrapings. Corneal abscess is a rare entity in patients with HIV and acquired immunodeficiency syndrome

Introducción: desde los primeros casos de virus de inmunodeficiencia humana (VIH), se conocen manifestaciones oculares secundarias a la infección y estas se han relacionado con el conteo de linfocitos CD4+. Objetivo: describir la correlación entre las manifestaciones oculares en pacientes con VIH y el conteo de linfocitos CD4+. Material y métodos: estudio transversal analítico de pacientes con VIH, en quienes se analizó la correlación entre conteo de CD4+ y manifestaciones oftalmológicas. Resultados: se incluyeron 21 pacientes entre 26 y 67 años de edad. Solo tres no se encontraban en terapia antirretroviral. El 67% presentó algún tipo de manifestación ocular, 42% presentó manifestaciones no relacionadas con la infección, 47% manifestaciones relacionadas y 24% ambas. La microangiopatía de la conjuntiva fue la manifestación ocular más frecuente (35.7%). Hubo una correlación estadísticamente significativa (r = 0.76, p = 0.0001) entre las manifestaciones oculares relacionadas con la infección y el conteo de linfocitos CD4+. Conclusiones: los pacientes con VIH presentan con frecuencia manifestaciones oculares, la mayoría asociadas a la infección. Existe correlación entre la presencia de estas con el conteo de CD4+; sin embargo, un número similar de manifestaciones no asociadas a la infección se presentaron sin correlación con el conteo, por lo que los pacientes con VIH deberían tener revisiones oftalmológicas periódicas, independientemente del conteo de CD4+.


Background: Since the first cases of human immunodeficiency virus (HIV), ocular manifestations secondary to infection have been known and these have been related to the CD4+ lymphocyte count. Objective: To describe the correlation between ocular manifestations in patients with HIV and the CD4+ lymphocyte count. Material and methods: Analytical cross-sectional study of patients with HIV whose CD4+ count was correlated with the presence of ophthalmological manifestations. Results: 21 patients between 26 and 67 years were studied. Only 3 patients were not on antiretroviral therapy. 67% of the patients presented some type of ocular manifestation, 42% presented non-infection related manifestations, 47% related manifestations and 24% both. Conjunctival microangiopathy was the most frequent ocular manifestation (35.7%). There was a statistically significant correlation (r = 0.76, p = 0.0001) between eye manifestations related to infection and CD4+ lymphocyte count. Conclusions: Patients with HIV frequently present ocular manifestations, the majority related to infection; there is a correlation between the presence of these with the CD4+ count. However, a similar number of manifestations not related to infection occurred without correlation with the count; therefore, HIV patients should have periodic ophthalmological examinations, independently of CD4+ count.

RESUMEN Las manifestaciones oculares de la artritis reumatoide son manifestaciones graves que requieren manejo agresivo. En el presente reporte presentamos un caso de queratitis asociada a derretimiento corneal desencadenado por suspensión abrupta de su medicación reumatológica y cirugía no ocular, que respondió de forma favorable al manejo con metil-prednisolona y ciclofosfamida.


ABSTRACT Ocular signs of rheumatoid arthritis are severe extra-articular manifestations, which usually require aggressive management. In this report, A case is presented here of patient with peripheral ulcerative keratitis associated with corneal melt syndrome triggered by abrupt suspension of antirheumatic medication and non-ocular surgery. They symptoms responded favourably to methylprednisolone and cyclophosphamide.

RESUMO No final do século vinte, com o surgimento de novas tecnologias e de novos programas espaciais, a medicina aeroespacial ganhou destaque no meio científico uma vez que os estudos relacionados às alterações da fisiologia humana no espaço tornaram-se cada vez mais necessário para a manutenção da saúde de cosmonautas. Os olhos são considerados uma das estruturas mais sensíveis do corpo às alterações vasculares, estruturais e bioquímicas provocadas pela microgravidade e radiação cósmica. Nesse sentido, essa revisão narrativa busca identificar e explicar as principais alterações morfológicas e funcionais que ocorrem no sistema visual em decorrência de missões espaciais.


ABSTRACT At the end of the twentieth century, with the emergence of new technologies and new space programs, aerospace medicine gained prominence in the scientific community since studies related to changes in human physiology in space have become increasingly necessary for the maintenance of cosmonaut health. The eyes are considered one of the most sensitive structures in the body to vascular, structural and biochemical changes caused by microgravity and cosmic radiation. In this sense, this narrative review seeks to identify and explain the main morphological and functional changes that occur in the visual system as a result of space missions.

RESUMO Objetivo: Analisar o perfil clínico de pacientes em acompanhamento dermatológico encaminhados para avaliação oftalmológica. Métodos: A amostra foi composta de pacientes dermatológicos encaminhados para avaliação oftalmológica, nos anos de 2016 e de 2017. Estudou-se a concomitância de doenças dermatológicas com as afecções oftalmológicas por meio da coleta simultânea do histórico dermatológico (prontuários registrados com dados gerais e diagnóstico) e de dados da consulta oftalmológica após o encaminhamento. Resultados: Foram avaliados pela oftalmologia 224 pacientes, sendo 65% do sexo feminino, 80% caucasianos, com idade variando entre 1 mês e 85 anos. As situações cujo encaminhamento foi mais prevalente foram psoríase, lúpus, vitiligo e rosácea (18,3%, 13,8%, 12,9% e 10,7%, respectivamente). Fototerapia crônica e uso de hidroxicloroquina representaram 35,7% e 22,3% dos pacientes. Casos de neurofibromatose, micose fungoide, líquen plano, neoplasias de pele, atopias, pênfigo e esclerodermia também estiveram presentes. Cegueira legal foi detectada em 6%, e deficiências visuais ligadas a afecções dermatológicas foram verificadas em 16,5% dos casos. As alterações oculares mais prevalentes foram catarata (18,9%), blefarite (15,9%), pterígio (5,3%) e conjuntivite (5,3%). Conclusão: Encontrou-se elevada frequência de alterações oftalmológicas em uma população de pacientes com doenças dermatológicas. Assim, o estudo e a análise de manifestações oculares em pacientes dermatológicos podem auxiliar na detecção precoce e na prevenção de complicações.


ABSTRACT Objective: To analyze the clinical profile of patients under dermatological care and referred to ophthalmological evaluation. Methods: The sample comprised dermatology patients referred to ophthalmological evaluation in 2016 and 2017. The simultaneous occurrence of skin and ophthalmic diseases was studied, by collecting dermatological history (medical records containing general data and diagnosis) and ophthalmic consultation data following referral. Results: A total of 224 patients were assessed, 65% were female, 80% were white, and age varied between one month and 85 years. The conditions more often referred were psoriasis, lupus, vitiligo and rosacea (18.3%, 13.8%, 12.9% and 10.7%, respectively). Chronic phototherapy and use of hydroxychloroquine were observed in 35.7% and 22.3% of patients, respectively. Cases of neurofibromatosis, mycosis fungoides, lichen planus, skin cancer, atopic dermatitis, pemphigus and scleroderma were also reported. Legal blindness was detected in 6% and visual impairment related to skin conditions in 16.5% of patients. The most prevalent ocular changes were cataracts (18.9%), blepharitis (15.9%), pterygium (5.3%) and conjunctivitis (5.3%). Conclusion: A high frequency of ophthalmic changes in a population of dermatological patients was found. In this context, studying and analyzing ocular manifestations in dermatological patients could be useful in early detection and prevention of complications.

ABSTRACT Introduction: The SARS-CoV-2 pandemic has been a major challenge for the international scientific community. Since its inception, studies aiming to describe pathophysiological aspects and clinical manifestations of the disease have been conducted, raising hypotheses and confirming possible associations. One aspect of this scientific medical production is the role of the ocular surface as a means of transmission and clinical presentation of viral syndrome. Objectives: To analyze the role of the ocular surface in transmission, pathophysiology, and clinical manifestations of SARS-CoV-2, by means of a systematic review. Methods: The search was carried out in three databases: Cochrane, PubMed Central Journals and MEDLINE, using the following descriptors: "COVID-19, ophthalmology". The filters last five years and studies on humans resulted in 32 studies; in that 12 were excluded for not meeting the purpose of the study. Results: There are still few published studies on the relation between SARS-CoV-2 and the ocular route. Most studies showed an association between the presence of nonspecific ocular manifestations and infection by the new coronavirus, with limitations in the number of patients analyzed and the methodology adopted. Hypotheses about the pathophysiological role are largely anchored in the association of SARS-CoV and the ocular surface evaluated in the past. Comments: The results found are still not sufficient to confirm the role of the ocular surface in the pathophysiology of the disease. Most of these preliminary studies are of considerable importance in raising hypotheses based on the medical analysis of the patients studied. However, larger studies with standardized methodology for diagnostic protocol and laboratory analysis of the individuals assessed are required.


RESUMO Introdução: A pandemia da SARS-CoV-2 tem sido um grande desafio para a comunidade científica internacional. Desde seu surgimento, estudos com a intenção de descrever os aspectos fisiopatológicos e as manifestações clínicas da doença vêm sendo conduzidos, levantando hipóteses e confirmando possíveis associações. Um dos temas dessa produção médica científica é o papel da superfície ocular como meio de transmissão e apresentação clínica da síndrome viral. Objetivo: Analisar o papel da superfície ocular na transmissão, na fisiopatologia e nas manifestações clínicas de SARS-CoV-2, através de uma revisão sistemática. Realizou-se a busca em três bancos de dados Cochrane Database, PubMed® e MEDLINE®, utilizando os descritores "COVID-19 e ophthalmology". Foram definidos como filtros o artigo ter sido publicado nos últimos 5 anos e estudo realizado em humanos, tendo sido encontrados 32 artigos. Destes, foram excluídos 12 por não corresponderem ao objetivo do estudo. Resultados: Ainda são poucos os estudos publicados sobre a relação entre o coronavírus 2 da síndrome respiratória aguda grave (SARS-CoV-2) e a via ocular. A maioria dos estudos mostrou associação entre a presença de manifestações oculares inespecíficas e a infecção pelo novo coronavírus, com limitações no número de pacientes analisados e na metodologia adotada. Hipóteses sobre o papel fisiopatológico se ancoram, em grande parte, na associação estudada entre o SARS-CoV-2 e a superfície ocular no passado. Comentários: Os resultados encontrados ainda não são suficientes para confirmar o papel da superfície ocular na fisiopatologia da doença. Grande parte desses estudos preliminares têm importância considerável ao levantar hipóteses baseadas na análise clínica dos pacientes estudados. No entanto, são necessários estudos maiores e com metodologia padronizada para protocolo diagnóstico e análise laboratorial dos indivíduos avaliados.

RESUMEN El trasplante alogénico de precursores hematopoyéticos está indicado en el tratamiento de diversos trastornos de la sangre, sobre todo en las neoplasias malignas. La enfermedad injerto contra huésped es la complicación principal de los trasplantes alogénicos de órganos hematopoyéticos. Es una enfermedad inmunológica provocada por la interacción entre el donante y el receptor, mediante respuestas innatas y adaptativas. Con gran frecuencia afecta la piel y el sistema gastrointestinal. Diferentes pueden ser las manifestaciones oculares, pero son las alteraciones de la superficie ocular las más frecuentes. Los cambios en la superficie ocular pueden provocar ceguera. El diagnóstico y el tratamiento precoz mejoran el pronóstico. Además del tratamiento general de la enfermedad es necesario instaurar medidas específicas para controlar las alteraciones oculares. Se realizó una revisión sobre los artículos publicados con el objetivo de conocer sobre la enfermedad injerto contra huésped y su afectación ocular(AU)


ABSTRACT Allogeneic transplantation of hematopoietic precursors is indicated in the treatment of various blood disorders, particularly malignant neoplasms. Graft-versus-host disease is the main complication of allogeneic transplants of hematopoietic organs. GVHD is an immunological condition caused by the interaction between the donor and the recipient manifested as innate and adaptive responses. It very often affects the skin and the gastrointestinal system. A variety of ocular manifestations may also occur, the most common of which are ocular surface alterations. Changes in the ocular surface may cause blindness. Early diagnosis and treatment improve prognosis. Besides the general treatment of the disease it is necessary to implement specific measures to control ocular alterations. A review was conducted of published papers on the topic to become acquainted with graft-versus-host disease and the ocular damage it causes(AU)

Introducción: El carcinoma de células renales representa el 2-3% de todos los cánceres. Alrededor del 30% de los pacientes presentan metástasis en el momento del diagnóstico y otro 30% de los pacientes con enfermedad localizada desarrollan recurrencia o metástasis. El sitio más común de metástasis es el pulmón, el hueso y el hígado. Objetivo: presentar una paciente con metástasis ocular como una presentación atípica de carcinoma de células renales. Caso clínico: Femenino de 41 años, tres meses previos inició con dolor ocular urente, fotopsias, hemianopsias y disminución de la agudeza visual del ojo derecho. La exploración física dirigida mostró tumor subretiniano. Se le realizó enucleación del ojo afectado y colocación de implante, con hallazgo de tumor intracelar y reporte histopatológico de carcinoma de células claras. Los estudios de extensión mostraron tumor renal izquierdo. Se le realizó nefrectomía radical izquierda con adrenalectomía derecha en un mismo tiempo quirúrgico. Discusión: El carcinoma de células renales metastásico tiene un pronóstico malo, la sobrevida al año es de 48% y a los 5 años de 9%. La metástasis única al momento del diagnóstico tiene pronóstico mejor que las múltijples. Conclusiones; El CCR puede metastatizar a sitios raros como el globo ocular entre otros (AU)


Introduction: Renal cell carcinoma accounts for 2-3% of all cancers. About 30% of patients have metastases at the time of diagnosis, other 30% with the patients with localized disease develop recurrence or metastasis. The most common site of metastasis are lungs, bones and liver. Objetive: to present a patient with ocular metastasis as an atypical presentation of renal cell carcinoma. Clinical case: Forty one years old female, three months before, began with burning eye pain, photopsies, hemianopsies and decreased visual acuity of the right eye. The directed physical examination showed subretinal tumor. Enucleation of the afected eye and implant placement was performed, with intracelar tumor finding and histopathological report of clear cell carcinoma. The extension studies showed left renal tumor. Left radical nephrectomy was performed with right adrenalectomy in the same surgical time. Discussion: Metastatic renal cell carcinoma has a poor prognosis, the survival rate is 48% at 5 years and 9% at 5 years. The single metastasis at the time of diagnosis has a better prognosis than multiple ones. Conclusions: RCC can metastasize to rare sites such as the eyeball among others (AU)

ABSTRACT

La incontinencia pigmenti es un trastorno neurocutàneo raro, con una frecuencia de 1 en 40 000 recién nacidos, de etiología genética asociada a mutaciones en el gen IKBKG, localizado en Xq28, con herencia dominante ligada al X. Tiene una presentación clínica de manifestaciones muy variables detectadas desde la etapa neonatal y puede asociar afectación cutànea, dental, ocular y neurológica, y cada una de estas con un diagnóstico diferencial distinto. Se presenta a una paciente pediàtrica con diagnóstico de incontinencia pigmenti a la semana de vida. En la evaluación oftalmológica inicial, se observaron lesiones vasculares retinianas. Se decidió el tratamiento con làser, con buenos resultados, y se consiguió estabilizar la visión.


Incontinentia pigmenti is a rare neurocutaneous disorder with a frequency of 1 in 40,000 newborn; it is associated with mutations in IKBKG gene in Xq28, inherited as an X-linked dominant trait. Clinical manifestations detected since the newborn period are highly variable, with skin, teeth, eyes, and nervous system manifestations, and each with a characteristic differential diagnosis. We present a pediatric patient diagnosed with incontinentia pigmenti at the first week of life. In the initial ophthalmologic evaluation, retinal vascular lesions were observed. The outcomes of laser treatment of the ischemic peripheral retina were good and resulted in stability of vision.

Resumo Objetivo: Conhecer a prevalência de manifestações oculares em trabalhadores portuários e identificar fatores associados. Métodos: Estudo transversal em um Porto marítimo do extremo Sul do Brasil. Participaram 232 trabalhadores. Para coleta dos dados utilizou-se entrevista estruturada. Foi utilizado teste t de Student para amostras independentes, a fim de comparar as médias dos trabalhadores com manifestações oculares, por meio dos qualificadores em grau no mínimo moderado (presença de manifestações oculares) e aqueles sem manifestações oculares ou grau leve (ausência de manifestações oculares). O teste de Mann-Whitney foi utilizado no caso de assimetria. O teste do qui-quadrado de Pearson ou o teste exato de Fisher foram usados para comparar proporções e a análise de Regressão de Poisson foi usada para controlar os fatores de confusão. Resultados: A prevalência de manifestações oculares autorreferidos foi 92,2%. As manifestações oculares foram: coceira/prurido (49,1%), irritação (41,4%), ardor (39,2%), lacrimejamento (34,9%), sensação de cansaço (32,3%), alteração na pigmentação ocular (26,7%), dor (17,7%), ressecamento dos olhos (14,2%) e sensação de pressão atrás dos olhos (9,9). A utilização de filme para envelopar mercadoria contribuiu para maior prevalência de manifestações oculares. Já a utilização de luvas como equipamento de proteção individual auxiliou na menor prevalências dessas manifestações. Conclusão: Acredita-se que os achados do presente estudo possam subsidiar a avaliação clínica do enfermeiro na abordagem de trabalhadores portuários, assim como direcionar o planejamento e a implementação de cuidados socioambientais condizentes com as necessidades de saúde desses e de outros trabalhadores.

La prueba de Mann-Whitney fue utilizada en el caso de asimetría. La prueba del Chi-cuadrado de Pearson o la prueba exacta de Fisher se utilizaron para comparar proporciones y el análisis de Regresión de Poisson se utilizó para controlar los factores de confusión. La utilización de una película para embalar mercancías contribuyó a una mayor prevalencia de manifestaciones oculares. La utilización de guantes como equipo de protección individual ayudó en la menor prevalencia de esas manifestaciones. Conclusión: Se cree que los hallazgos del presente estudio pueden subsidiar la evaluación clínica del enfermero en el abordaje de trabajadores portuarios, así como dirigir la planificación y la implementación de cuidados socioambientales que estén de acuerdo con las necesidades de salud de esos y de otros trabajadores.


Abstract Objective: To understand the prevalence of ocular manifestations in port workers and identify associated factors. Methods: Cross-sectional study carried out in a seaport in southern Brazil. A total of 232 workers participated in the study. Structured interviews were applied for data collection. Student's t-test was used for independent samples to compare the means of workers with ocular manifestations, applying qualifiers with a moderate minimum level (presence of ocular manifestations) and with a low level or without ocular manifestations (lack of ocular manifestations). The Mann-Whitney test was used for cases of asymmetry. Pearson's Chi-square and Fisher's exact test were carried out to compare the rates, whereas Poisson Regression was used to control the confounding factors. Results: The prevalence of self-reported ocular manifestations was 92.2%. Ocular manifestations were: itching/pruritus (49.1%), rash (41.4%), burning (39.2%), lacrimation (34.9%), fatigue (32.3%), change in ocular pigmentation (26.7%), pain (17.7%), eye dryness (14.2%), and pressure sensation behind the eyes (9.9%). The use of film for wrapping goods contributed to a higher prevalence of ocular manifestations. The use of gloves as personal protective equipment led to a lower prevalence of these manifestations. Conclusion: The findings of the current study may contribute to the clinical assessment of nurses in approaching port workers, as well as to guide the planning and implementation of social and environmental care in line with the health needs of these or other workers.

ABSTRACT

RESUMEN Se presenta a un paciente que ingresó en el antiguo Hospital Provincial Clínico Quirúrgico Docente "José Ramón López Tabrane", de Matanzas. El motivo del ingreso fue una proptosis ocular derecha de instauración súbita en el curso de la infección-enfermedad por VIH-sida. Se le realizaron múltiples estudios para poder definir la causa, y, pese a imponer tratamiento oportuno, falleció a los 15 días del mismo. La epidemia de sida continúa creciendo mundialmente. El conocimiento de las manifestaciones oculares; tanto las alteraciones del segmento anterior como las del posterior, y las infecciones asociadas, permiten mayor posibilidad de preservar la función visual en estos pacientes (AU).


ABSTRACT We present the case of a patient who entered the former Provincial Teaching Clinical Surgical Hospital "José Ramón López Tabrane", of Matanzas. The cause of admission was a sudden right ocular proptosis in the course of an HIV-AIDS infection-disease. Many different studies were carried out to define the cause, and although he was timely treated, he died 15 days after the treatment. The AIDS epidemic is still growing around the world. The knowledge of ocular manifestations, the alterations of the anterior segment as much as the alterations of the posterior one, and the associated infections allow a higher possibility of preserving the visual function in these patients (AU).

Le syndrome de Stevens-Johnson et le syndrome de Lyell, ou nécrolyse épidermique toxique, représentent des réactions d'hypersensibilité cutanées rares, habituellement d'origine médicamenteuse, potentiellement mortelles. Les atteintes ophtalmologiques au cours de ces syndromes peuvent être graves et conduire à la cécité. Ces atteintes peuvent être prises en charge par des soins oculaires appropriés. Nous rapportons deux cas de syndrome de Lyell: le premier ayant présenté une complication oculaire due à l'absence de la prise en charge oculaire en phase aigüe. ABSTRACT

La incidencia de la leucemia linfocítica crónica (LLC) aumenta progresivamente con la edad; aproximadamente el 75 por ciento de los casos presentan 60 años o más. Este tipo de leucemia es más frecuente en varones y se desconoce su causa, existen casos que son de origen hereditario. Se presenta el caso de un paciente con el diagnóstico de LLC con infiltración ocular. Paciente blanco, masculino de 76 años de edad con antecedentes de salud de haber sido diagnosticado con LLC desde hace 5 años que se trata con Leukeran (Clorambucil) (2mg) 2 tabletas en el almuerzo y 3 tab en la comida, así como de prednisona (5mg) 1 cada 8 horas solo cuando está descompensado. Hace alrededor de 3 días comenzó con astenia, anorexia, mareos, dolor e inflamación del párpado superior derecho. El examen físico, la biopsia del párpado superior y el frotis de sangre revelaron la presencia de una recaída hematológica de la LLC con infiltración ocular(AU)


The incidence of chronic lymphocytic leukemia (CLL) increases progressively with age, approximately 75 percent of cases present 60 years or more. This type of leukemia is more frequent in men and its cause is unknown, there are cases that are of hereditary origin. A clinical case of a patient with the diagnosis of Chronic Lymphocytic Leukemia with ocular infiltration is presented. A 76-year-old white male patient with a health history of having been diagnosed with a Chronic Lymphocytic Leukemia for 5 years who is treated with Leukeran (Chlorambucil) ( 2mg) 2 tab at lunch and 3 at night; and prednisone (5mg) 1 tab every 8 hours. About 3 days ago begins with asthenia, anorexia, dizziness, pain and swelling of the upper right eyelid. Physical examination, upper eyelid biopsy and blood smears reveal the presence of a hematological relapse with ocular infiltration(AU)

RESUMEN Se informa el caso de una paciente con enfermedad de Gaucher (EG) tipo 3b con mutación homocigota en el gen GBA (c.1448T>C p.L483P) (L444P). Se describen los hallazgos oculares característicos de esta mutación, que incluyen condensaciones vítreas y edema macular. Hasta donde sabemos, es el primer caso informado en Colombia con estas características. Se presenta además una revisión sobre las manifestaciones oculares de esta enfermedad.


SUMMARY We report the case of a patient with Gaucher disease (GD) type 3b, with a homozygous GBA gene mutation (c.1448T > C p.L483P) (L444P). Ocular findings characteristic of this mutation are described, including vitreous condensation and macular edema. To our knowledge this is the first case reported in Colombia with these characteristics. A review of the ocular manifestations of this disease is also presented.


RESUMO Se informa o caso de uma paciente com doença de Gaucher (EG) tipo 3b com mutação homozigoto no gene GBA (c.1448T>C p.L483P) (L444P). Se descrevem as descobertas oculares características desta mutação, que incluem condensações vítreas e edema macular. Até onde sabemos, é o primeiro caso informado na Colômbia com estas características. Se apresenta ademais uma revisão sobre as manifestações oculares desta doença.

Resumo Objetivo: A doença de Behçet é uma vasculite inflamatória sistémica, de etiologia desconhecida. Pode atingir virtualmente todos os sistemas, sendo as manifestações oculares comuns. O objectivo deste trabalho foi analisar doentes com doença de Behçet ocular quanto aos parâmetros demográficos, manifestações clinicas, terapêutica e principais complicações. Métodos: Estudo descritivo e retrospetivo, que incluiu 11 doentes com o diagnóstico de doença de Behçet ocular, segundo os critérios do ISG, observados na consulta de Inflamação Ocular do Hospital Prof. Doutor Fernando da Fonseca nos últimos 3 anos. Resultados: Identificaram-se 11 doentes, 5 homens e 6 mulheres, todos caucasianos. A idade média ao diagnóstico foi de 33,45±6,49 anos. A manifestação ocular foi o primeiro sinal da doença em 2 doentes. Em 72,7% dos casos as manifestações oculares foram bilaterais. Identificaram-se 4 casos de panuveíte, 3 de uveíte posterior, 2 de uveíte anterior, 1 de queratite e 1 caso de episclerite. O glaucoma e a catarata foram a complicação ocular mais frequente. O tratamento sistémico incluiu a corticoterapia oral em associação com terapêutica adjuvante imunossupressora, sendo os mais utilizados a azatioprina e a ciclosporina. Em 3 doentes houve necessidade de terapêutica biológica com infliximab para controlo da doença. Conclusão: A manifestação ocular mais frequente foi a panuveíte. Esta doença pode condicionar complicações oculares com diminuição irreversível da acuidade visual. A orientação destes doentes exige uma abordagem global e interdisciplinar.


Abstract Objective: Behçet's disease is a systemic inflammatory vasculitis of unknown etiology. It can virtually reach all systems, being common ocular manifestations. The aim of this study was to analyze patients with ocular Behcet's disease regarding demographic parameters, clinical manifestations, therapeutic approach and main complications. Methods: Descriptive and retrospective study, including 11 patients diagnosed with ocular Behcet's disease, according to the ISG criteria, observed in the Ocular Inflammation appointment of the Hospital Prof. Doutor Fernando da Fonseca in the last 3 years. Results: We identified 11 patients, 5 males and 6 females, all caucasian. The mean age at diagnosis was 33.45 ± 6.49 years. Ocular manifestation was the first sign of the disease in 2 patients. In 72.7% of the cases, ocular manifestations were bilateral. There were four cases of panuveitis, 3 of posterior uveitis, 2 of anterior uveitis, 1 of keratitis and 1 case of episcleritis. Glaucoma and cataract were the most frequent ocular complications. Systemic treatment included oral corticosteroid therapy in combination with adjuvant immunosuppressive therapy, and the most commonly used was azathioprine and cyclosporine. In 3 patients there was a need for biological treatment with infliximab to control the disease. Conclusion: The most frequent ocular manifestation was panuveitis. This disease can promote eye complications with an irreversible decrease in visual acuity. Orientation of these patients requires a global and interdisciplinary approach.

Objetivo: describir el comportamiento de manifestaciones oftalmológicas de anexos y segmento anterior en pacientes diabéticos de un área de salud. Métodos: se realizó un estudio descriptivo transversal. La muestra fue conformada por 134 pacientes diabéticos. Se aplicaron métodos de estadística descriptiva para variables cualitativas. Para el procesamiento y análisis de la información se confeccionó una base de datos en programa estadístico Microsoft Excel 2003. Resultados: el 88,8 por ciento de los pacientes tenían más de 50 años. Predominaron el grupo de 60-69 años, con edad media de 62,5 años; el sexo femenino a razón de 97:37 y la diabetes mellitus tipo 2 (97,8 por ciento), con menos de 5 años de evolución (57,4 por ciento). Las enfermedades oculares que se presentaron con mayor frecuencia fueron ojo seco (76,1 por ciento), blefaritis (41,7 por ciento), pterigion (20,1 por ciento), tortuosidad vascular conjuntival (15,6 por ciento), engrosamiento conjuntival (11,1 por ciento), queratitis punctata y neurotrófica (4,5 por ciento) seguida de herpes zoster oftálmico (2,2 por ciento), rubeosis del iris (1,5 por ciento) y úlcera corneal y catarata (0,8 por ciento). Conclusiones: la diabetes mellitus es un problema de salud real. Las personas diabéticas presentan con frecuencia daño ocular como ojo seco, blefaritis, queratitis, entre otros, por lo que deben consultar al oftalmólogo periódicamente(AU)


Objective : to describe the behavior of the ophthalmologic manifestations of annexes and previous segment in diabetic patients in an area of health. Method: a carried out a traverse descriptive study. The sample was constituted by134 diabetic patients. Methods of descriptive statistic were applied for qualitative variables. For the prosecution and analysis of the information a database was made in statistical program Microsoft Excel 2003. Results: 88,8 percent of the patients had more than 50 years prevailing the 60-69 year-old group, with half age 62,5 years old. The feminine sex prevailed to reason of 97:37. Diabetes mellitus type 2 prevailed (97,8 percent), with less than 5 years of evolution (57,4 percent). The ocular pathologies that were presented with more frequency were dry eye (76,1 percent), blepharitis (41,7 percent), pterigion (20,1 percent), tortuosity vascular conjunctival (15,6 percent), and conjunctival engrosamiento (11,1 percent). Punctata keratopathy and neurotropic keratopathy (4,5 percent) followed by herpes zoster ophthalmicus (2,2 percent), rubeosis of the iris (1,5 percent) and corneal ulcer, waterfall (0,8 percent). Conclusions: the diabetic mellitus is a problem of real health. Diabetic people frequently present ocular damage as dry eye, blepharitis, keratopathy among other, for what they should attend the oculist periodically(AU)

ABSTRACT

PURPOSE: To describe the characteristics and frequency of ophthalmologic findings in patients with Lennox-Gastaut syndrome (LGS). METHODS: The medical records of patients diagnosed with LGS at Seoul National University Children's Hospital from January 2004 to August 2014 were retrospectively reviewed. The records of 34 patients (mean age ± standard deviation, 2.66 ± 3.51 years; male, 58.8%) were reviewed. The primary measure was the incidence of ophthalmologic manifestations. RESULTS: Of the 34 patients, 88.2% had at least one ocular abnormality. Refractive error (52.9%) was the most frequently observed ophthalmologic manifestation in patients with LGS, followed by strabismus (32.4%), cortical visual impairment (23.5%), and retinopathy of prematurity (8.8%). Among these cases, seven patients had exotropia and three had esotropia. CONCLUSIONS: LGS is a childhood-onset epileptic encephalopathy with variable ophthalmologic manifestations, the most frequent being refractive errors. Patients with suspected LGS should be examined regularly because ophthalmological features can change during their disease course.
